Community Programs:

Sometimes people think hospice palliative care only applies in the last days or weeks of life, but Hospice Caledon services and programs are available for people from the time of diagnosis, walking with them on the journey through their illness.  We offer one-on-one support by trained qualified staff, group programs, as well as, Visiting Volunteers who provide support in the home. We are also there for families – providing support for the caregiver, respite and offering grief support before and after the loss of a loved one. The following describe some of the programs that Hospice Caledon currently provides:

In-Home Visiting Volunteer Program
Hospice Caledon’s specially trained In-Home Volunteer Visitors provide in-home supportive care for individuals with life-limiting illness and their families.  In-Home Volunteer Visitors provide emotional, practical, social and spiritual support to people they visit.  Volunteer Visitors will not provide nursing or medical care, give medications, or offer advice or counselling.

Wellness Program
This is a weekly afternoon program that is held in a caring, friendly and safe setting.  This is an afternoon set aside for healing and enrichment.  The programs offered are designed to introduce new interests and lifestyles.  This program is available to both clients and their families.

Grief Support Program
Hospice Caledon offers Bereavement Support Group programs to the community and to the family members of our clients.  We hold periodic 10-week support group programs and a regular monthly drop-in grief support group.

Let's Talk Program
A relaxed sociable setting giving opportunity for people to connect to other people who have lost loved ones. Meetings held on the last Thursday of each month at various locations. Please call for more information.

To inquire about these programs and services or to register for the bereavement support group contact our program manager, 905-951-3534.
