What is Hospice Palliative Care?

Palliative care is aimed at relief of suffering and improving the quality of life for persons who are living with or dying from advanced illness or are bereaved.

Canadian Hospice/Palliative Care Association

While Hospice Palliative care originated as a movement to provide compassionate end-of-life care, it has currently taken on a much broader role.  Today Hospice Palliative Care is open to any person who feels they require support because of a life challenging illness.

Hospice Services

We offer a wide range of professionally staffed, volunteer supported services including:

Hospice Caledon works closely with other care providers in our community including physicians, hospitals, home nursing, long term care facilities, therapists, faith community, nutritionists, pharmacists, etc.to provide seamless services to meet our patients' and their familes' needs and expectations for quality of life.

When is Hospice Palliative Care appropriate?

Ideally, the patient will be referred or come to Hospice palliative care around the time of initial diagnosis.  Hospice's caring professional staff will coordinate assessment of the patient and the family's needs and expectations during this difficult time and assist in sharing information, providing or referring to needed services and advocacy.  Patients and families may choose any of Hospice Caledon's professional and volunteer based services, depending on individual needs.  This individualized plan of care will change and grow as the patient's and family's needs change.

Hospice Caledon's Community Hospice Programs are made possible largely through the generous donations of time, goods and funds by our local citizens, community organizations and businesses, which supplement partial funding from the Ministry of Health and Long Term Care.
